Family Case Management
Family Case Management extends services for children ages birth to two years of age.
Services include physical assessments, nutritional counseling, prenatal education, case management, referrals to WIC, immunizations, well- child care, early intervention, and limited financial assistance for income- eligible pregnant women.

* For families larger than eight, add $623 for each additional person.
FCM offers pregnancy testing to childbearing teens and women
by appointment. If the test is positive, the pregnant teen or
woman is connected to a Public Health Nurse who will serve as
her case manager. The Public Health Nurse will assist the
mother-to-be in obtaining a variety of services involving the whole
family.
Services available to FCM clients include medical coverage
(medical card or All Kids); nutrition education and assistance
(WIC); mental health; education (school or GED/ childbirth/
breastfeeding/ child development/ special referrals); and
others.

These services are also provided to DCFS wards up to age 6 who
are placed in Wayne County and to families referred by area
hospitals to the Adverse Pregnancy Outcome Reporting System (APORS).
WCHD serves as an outstation site for completion of All Kids
applications.
Our Case Managers
All assessments are either conducted in the client's home or at the Wayne County Health Department. Information is readily available, and the Case Manager will provide consultation about other services available in Wayne County.
Case Managers can provide advocacy, referral to a physician, and coordination of other services.
