Family Case Management
This program is designed to assure adequate maternal and infant care during the prenatal, delivery, and early post-partum periods.
Family Case Management extends services for children ages birth to two.
Services include physical assessments, nutritional counseling, prenatal education, case management, referrals to WIC, Immunizations, Well Child Care, Early Intervention, and limited financial assistance for income eligible pregnant women.

If the test is positive the pregnant teen or woman is connected to a Public Health Nurse who will serve as her case manager. The Public Health Nurse will assist the mother-to-be obtain a variety of services involving the whole family.
Services available to FCM clients include medical coverage (medical card or Kid Care); nutrition education and assistance (WIC); mental health; education (school or GED/ childbirth/ breastfeeding/ child development/ special referrals); and others.

These services are also provided to DCFS wards up to age 6 who are placed in Wayne County and to families referred by area hospitals to the Adverse Pregnancy Outcome Reporting System (APORS).
WCHD serves as an outstation site for completion of KIDCARE applications.
Our Case Managers
Through the Family Case Management Program, educated and trained Case Managers who are Registered Nurses will complete a comprehensive assessment to identify the needs of the family.
All assessments are either conducted in the client's home or at the Wayne County Health Department. Information is readily available, and the Case Manager will provide consultation about other services available in Wayne County.
Case Managers can provide advocacy, referral to a physician, and can help coordinate other services.
